After an in home assessment, we develop a personalized care plan based on your loved one’s needs and provide ongoing support to help them maintain a healthy, positive lifestyle.
We can assist with wound care, catheter care, oxygen therapy, medication management, colostomy care, diabetes support, mobility concerns, and more. Families also receive updates after care is provided by phone, text, or email.

A Senior Independence Assessment is a comprehensive in-home evaluation designed to help older adults remain safe, confident, and independent in their own homes for as long as possible.
During this visit, a healthcare professional will assess key areas that influence a person’s ability to live independently. This includes mobility and fall risk, medication routines, cognitive changes, home safety, and overall daily functioning.
The goal is to identify concerns before they become serious problems. Many avoidable hospital visits and transitions to long-term care begin with issues that could have been addressed earlier.
Following the assessment, you will receive a report of the findings along with practical recommendations. These may include strategies to improve safety, suggestions for home adaptations such as grab bars or mobility supports, medication organization, or preventative monitoring plans.
